One way to do this is to check into a halfway home. A halfway home increases your chances of maintaining a sober living lifestyle. While it is not as structured as the rehab you may have just left, it's doesn't leave you on your own at home. You'll be accountable for your choices and you'll have other people who are going through the same things you are living with you. You won't be tossed back into your old lifestyle, where you'll have old haunts and old friends surrounding you, and tempting you. The time you spend in one of these houses gives you the extra time outside of rehab that you need to reinforce all that you've learned.
